Serum Hyaluronic Acid and Type IV Collagen Levels in Patients with Chronic Liver Diseases


Abstract
There was recently reported that serum levels of hyaluronic acid and type IV collagen are usuful serological markers in noninvasively detecting the degree of hepatic fibrosis and in the differential diagnosis of chronic hepatitis (CH) and liver
cirrhosis(LC). But the clinical signficance about these is not clearly established. To evaluate the changes of serum hyaluronic acid and type IV collagen levels in chronic liver diseases and whether these loves are useful serum markers in the
differential diagnosis of chronic active hepatitis(CAH) and LC or not, we investigated serum levels of hyaluronic acid and type IV collagen in 28 patients with CAH, 24 patients with LC, 15 patients with hepatocellular carcinoma(HCO) and 23
healthy
adult
subjects as control. The serum hyaluronic acid level was measured by sandwich binding protein assay, and serum type IV collagen levels was measured by one-step sandwich enzyme immunoassay. Serum levels of hyaluronic acid and type IV collagen were
found
to be significantly increased in CAH, LC, and HCC compared with healthy subjects, and in LC and HCC compared with CAH. At the level of 175 ng/ml for cut off value of hyaluronic acid and 230 ng/ml for type IV collagen, the diagnostic efficiency of
LC was
86.5% and 82.7% and the specificity was 85.7% and 82.1%, respectively. When cut off value of hyaluronic acid was evaluated in combination with that of type IV collgen (230 ng/ml), its diagnostic efficiency and screening capabilities were
enhanced.
From
these results, it is suggested that serum levels of hyaluronic acid and type IV collagen could be useful serological markers in differentiating CAH from LC.
